Tesla driver overrode self-driving in fatal Texas home crash, NTSB says

A 76-year-old woman was killed in June after a 2025 Tesla Model 3 plowed into her home at high speed in Katy, Texas. The vehicle was traveling at more than 70 mph when the crash occurred.

In a preliminary report released Wednesday, the National Transportation Safety Board concluded that the driver manually overrode the vehicle's advanced driver assistance system. While the car was operating on Full Self-Driving (Supervised), the driver slammed the accelerator pedal to 100 percent.

Federal investigators determined that the driver overrode the driver assistance mode by fully pressing the accelerator, causing the vehicle to accelerate into the building.
